In-Office Coverage Plan
Adult Plan: $425/Year
- Comprehensive exam for new patients.
- Two six-month check-ups.
- Two cleanings.
- Fluoride varnish.
- All routine x-rays (bitewings, PA’s, panoramic).
- Emergency exams and x-rays during business hours.
- 15% off cash price of advanced treatments, including fillings, crowns, extractions, and implants.
- $500 discount on SureSmile clear aligners.
Periodontal Plan: $575/Year
- Comprehensive exam for new patients.
- Two six-month check-ups.
- Four cleanings.
- Fluoride varnish.
- All routine x-rays (bitewings, PA’s, panoramic).
- Emergency exams and x-rays during business hours.
- 15% off cash price of advanced treatments, including fillings, crowns, extractions, and implants.
- 15% off cash price of deep cleanings (SRP/scaling, root planing, etc.).
Additional Family Members
- Kids (ages zero through 14): $325 each.
- Family members (ages 15 and older): $375 each.
Terms & Conditions
- Annual enrollment fees are non-refundable, even if all plan benefits are not used within the coverage period.
- Payment for enrollment must be made in full at the start of the plan.
- Coverage begins on the first date of use and is valid for one year from that date. Plans are only available on an annual basis.
- Costs for dental treatments must be paid in full at the time of service.
- Exams, x-rays, cleanings, and fluoride applications must occur during the enrollment year and cannot roll over to the following year.
- Patients or parents are responsible for scheduling and keeping appointments. Missed appointments without 24-hour advance notice may incur a fee.
- Coverage plans cannot be transferred to other individuals or uncovered family members.
- Discounts cannot be combined with any other offers, plans, or existing insurance.
- Payments made through CareCredit’s extended payment plan may reduce the discount rate by 5% to offset merchant fees.
- Fees are based on the practice’s usual, customary, and reasonable rates, which are subject to change.
- Annual enrollment fees and coverage terms are subject to updates.
- Plans cannot be used for treatments covered under Workman’s Compensation.
- Discounts do not apply to services provided by specialists outside the practice.
- Coverage remains valid only for participants who stay enrolled through the completion of their treatment plan.
- Discounts apply solely to services and do not include materials or products.
